Application form : prescribed OMR application forms and details can be purchased from all main / Head post offices in all the district of UP from 18/07/2013 till last date i.e. 20/08/2013 on payment of Rs. 200/-

How to Apply : application forms complete in all respect should be submitted in the same post office from where the form was purchased till 5.00 pm 20/08/2013
